Moderna Boosters are approved for anyone 18 years and older who have completed a primary COVID-19 vaccine series and has been 2 months or more since their last dose.

* You must complete the Pre-Vaccination Checklist form:

* 1. Select which vaccine you wish to receive

* 2. Please select appointment date and time

* 3. Complete ALL fields on the form

* 4. Sign and date the form

* 5. Please submit

Please wear a face covering and be ready to wait for at least 15 minutes post vaccination for observation. Due to this requirement, we are not able to do curbside vaccine administration.

Vaccines are free of charge but please bring insurance/Medicare card if insured or Drivers License/Social Security card if un-insured.

Please call the store if you need to cancel your appointment.
